All about your first physical therapy appointment

Every patient has a unique health history, diagnosis, and personal goals. When you come to your first physical therapy appointment at Benchmark, we’ll work together to create a personalized treatment plan for you. Your goals are our goals.

When to arrive
On average, your first physical therapy appointment will last about an hour. Please arrive 15 minutes early to sign in, complete paperwork, and change clothes, if necessary.

What to bring
At your first physical therapy appointment, you’ll need to bring your physician referral or prescription (if applicable), your insurance card, registration forms, ID, and co-payment (if applicable). You may also bring a change of clothes.

How it works
During your appointment, your physical therapist will complete an initial evaluation and discuss your care plan. They’ll also work with you to make a plan for your continued treatment. Your physical therapy goals may include improved movement, strength, endurance, and flexibility, as well as decreased pain.

Keeping you safe

Patient and staff safety is a top priority at Benchmark Physical Therapy. See how we’re protecting your safety:

  • All patients and staff are screened for symptoms of COVID-19, including temporal temperature scans, each day upon arrival.
  • All staff and patients are required to wear face masks.
  • Clear plastic shields in the check-in area separate patients from the staff.
  • Only patients are permitted in the waiting area. Guests will be asked to remain in their vehicles.
  • We practice recommended social distancing guidelines when not delivering hands-on care.

Insurance & billing

We work with most major insurance providers and do our best to help keep the paperwork pain-free. If you have questions about physical therapy insurance coverage, please ask when you schedule your appointment.

If your insurance provider requires a co-pay, we will ask for payment at each visit. We accept cash, check, and credit card.
